To facilitate tracking and expedite your order, items are assigned a Return Material Authorization Tracking number, after completing this form and supplying the following information. To readily identify your package, display this RMA Tracking number on the shipping label, and include it on all corresponding documentation.
Matthews Marking Systems provides a flat rate repair for Ink-Jet Printheads and Ink Systems. We will refurbish your equipment and get it back into service with you as quickly as possible. All this work including parts and labor is covered by one low, flat rate! The anticipated lead time is two weeks from receipt of equipment. A valid purchase order or credit card is required.

- IN WARRANTY (within 1 year of ship date)
- Matthews will replace the controller and all of the installed licenses free of charge as long as there is no physical damage to the controller. In the case of physical damage, Matthews' customers will need to purchase a new controller and the licenses will then be transferred to it free of charge. We will need the original Machine ID and serial no. The Machine ID is on the back of the controller.
- If immediate replacement is needed, please provide a new purchase order for the controller and all required licenses. Email the P.O. to MMS-insidesales.com. Obtain an RMA# and send the unit back. Matthews will credit your account after we receive the defective controller back, provided there is no physical damage.
- OUT OF WARRANTY
- If there are no licenses on your controller, you will need to purchase a new controller. We do not need the failed unit sent back as we do not repair controllers. Email your PO to MMS-insidesales.com.
- If licenses have been installed on the controller, complete the information on this form to obtain an RMA No. Send the unit back and email your PO to MMS-insidesales.com. After we receive the failed unit back, all licenses from the failed unit will be transferred to the new one prior to shipment. Only a new controller will be billed, and Matthews will transfer your licenses free of charge. We will need the original Machine ID and serial no. The Machine ID is on the back of the controller.

MPERIA® Controllers, Lasers and C-84's repairs will require estimates. Quotations will be provided one week after receipt of the product. A valid Purchase Order will be required to complete repair. The cost determined upon evaluation of the equipment and estimated repair lead time will be included in the quotation.
